Am I a Candidate for Bariatric Surgery?
As you determine if bariatric surgery is the right choice for your needs, you may wonder if you qualify for weight loss surgery. Most people with a BMI greater than 35 will qualify for surgery.
If you still need to meet the listed criteria for surgery based on your health insurance coverage, Dr. Bagnato can still help. Contact us, and we will provide all relevant medical information to your insurance carrier.
Your First Visit
Before you visit our office, please complete several forms to make your first visit easier. When you contact us to schedule an appointment, we will provide you with a “patient access code.” Please enter this code and your last name into our patient portal to begin this process.
During your initial consultation, Dr. Bagnato will review your medical history and conduct a complete medical assessment. If your insurance company rules it necessary, we will also complete a nutritional and mental health assessment.
Please bring the following documentation to your first visit:
- Current insurance card
- Valid picture ID
- Medical records covering primary care for the past 3-5 years
- Documentation of past weight loss efforts
- Printed list of current medications from your pharmacy
Please do not fax any documents to our office. Submit paperwork using our online portal, and bring accompanying documentation to your first visit.
